2025 Required Pre-College Program Forms
Below you will access the forms that all students/parents must complete and digitally submit prior to attending 91°µÍø's Pre-College Program. Scholarship Application form deadline is March 15, 2025. All other forms are due by June 1.
Application Forms
Consent and Waiver Release (PDF or Word Doc) – uploaded to the online application
Residential and Academic Life Agreement - checkbox agreement included in the online application
Scholarship Application (PDF or Word Doc)– uploaded (as applicable) to the online application.
Internal Tuition Grant application (available to dependents of 91°µÍø faculty and staff only) * submit directly to 91°µÍøâ€™s Human Resources department following online Pre-College
application submission.
Accepted Student Forms
Minors Health Form (requires a physician’s authorization, and also an office visit if a physical examination
has not occurred since 7/1/2024). Upload digitally .
Student Transportation Form – Residential Students - Upload digitally . Commuter Students complete online form .
2025 Pre-College Program Policies
Students must abide by the academic rules of conduct established by 91°µÍø. The student signing this contract affirms the intention to abide by the terms of the 91°µÍø Honor Code, as follows in part: "I hereby accept membership in the 91°µÍø community and, with full realization of the responsibilities inherent in membership, do agree to adhere to honesty and integrity in all relationships, to be considerate of the rights of others, and to abide by the college regulations." Cheating or plagiarism in any Summer Session class may be cause for dismissal from the program. All members of the 91°µÍø community, including students, faculty, and staff, are parties to the honor contract and are expected to abide by its provisions. Students must maintain a high level of respect for each individual in the program and in the 91°µÍø community at all times. Verbal, written, sexual, or physical harassment/harm of any kind to any person including self will not be tolerated and may be cause for dismissal from the program.
